Kinect Australia and Andrology Australia (Monash University) are conducting a physical activity and health program within your local government. This project is funded by the Australian Department of Health and Ageing. Your council has agreed to participate and promote this program.

Program details
Participation is voluntary and you are under no obligation to consent to participation. If you agree to participate you may withdraw your consent at any time, either by notifying me by phone or in writing. Not participating will not disadvantage you in any way. As with any physical activity program, there is a small risk of injury associated with increasing physical activity levels. We will work with you to minimise these risks by providing information on how to minimise risk, answering questions from you regarding any concerns you might have. All participants are required to complete a form that will assess risk before commencing. All employees are eligible. No personal information is released to your employer.

Research details
No findings, which could identify any individual participant, will be published. Your anonymity is assured by our procedures, in which the questionnaires are anonymous and only the combined results of all participants will be published. A report of the study may be submitted for publication, but individual participants will not be identifiable in such a report. You may access your record at any time, by contacting us. Published research will be made available to your employer to distribute or you may contact us to receive this directly. Storage of the data collected will adhere to University regulations and kept on University premises in a locked cupboard/filing cabinet for 5 years. We will collect data on the assessments that you complete, visits to the website and telephone health coaching uptake.

We reserve the right to send you certain communications relating to the project. If you do you not wish receive e-mails and newsletters, please let us know by contacting us. Also, please contact us to access and update or delete your information.
